About Us:
The RGP supports health care professionals in Toronto and surrounding regions in the provision of interdisciplinary, senior-friendly, and evidence-based care that optimizes the function and independence of seniors and their ability to age in place.
The RGP supports a network of 26 participating organizations whose services span the continuum of hospital and community based care. See our network map and the list of our participating organizations.

The RGP builds capacity by making clinical tools and educational resources available to practitioners.
