Interim HealthCare San Diego Hospice

We are a Medicare and Medical certified, CHAP accredited hospice delivering extraordinary, individualized patient care in the home setting throughout all of San Diego County. We were licensed and accredited in 2011 and have now cared for over 1500 patients and their families. Our interdisciplinary team of professionals includes a hospice physician, registered nurse case managers, licensed vocational nurses, hospice aides, social workers, chaplains, homemakers, and volunteers. We practice our mission of providing patient and family empowered care, preserving dignity and providing comfort in each of our actions and decisions.
