CHP Home Care & Hospice

CHP Home Care and Hospice provides home health, hospice, and related services throughout Northwest and West Central Ohio. Founded in 1975, CHP is a nonprofit agency with office locations in Ada, Archbold, Bryan, Celina, Defiance, Delphos, Lima, Paulding, Van Wert, and Wapakoneta, Ohio.
