Report of Marriage
 
1.    Personal appearance of both contracting parties
 
2.    Four (4) copies of typewritten Report of Marriage Form with original signature
● Please use black ink pen only
● Entries must be in CAPITAL LETTERS
● Erroneously filled-out forms will NOT be accepted
 
3.    Original and four (4) photocopies of the Marriage Contract (with English translation) attested by the UAE Ministry of Foreign Affairs
 4.    Four (4) pieces passport size photos of each of the contracting parties
5.    Four photocopies each of the contracting parties’ valid passports
 6.    Affidavit of Delayed Registration (If applicable)
 7.   Negative Certification of Marriage Record from PSA (if applicable)

 8.    Other necessary requirement/s requested by the processor to determine proof of
citizenship, identity or eligibility for registration of marriage under Philippine Laws.

Payment: cash
    ● 100AED for attestation
   ● 100AED for Report of Marriage
   ●   100AED for Affidavit of Delayed Registration (if applicable)
   ●   400AED for notarization

The Report of Marriage form must be signed by the informant/s at the bottom using black ink pen only.

Please note that this is out of town marriage and registration may take minimum of six months onward before you can get your copy as we will be sending it to Ankara Philippine Embassy if you got married in Tbilisi Georgia
