Leaders in medicine learn from the errors of others in order to prevent the same mistakes. It is recommended to fix the cause of the error, rather than resorting to a "culture of shame and blame." Physician leaders should turn sources such as The Doctors Company's patient safety Web site, the Joint Commission, the National Patient Safety Foundation, and the PDR Network to study patient safety.
