রোগ নির্ণয়:
PACE হাসপাতালে ভর্তির সময়, রোগীর পেটে CT করা হয়েছিল এবং একাধিক প্যারেনকাইমাল ক্যালসিফিকেশন সহ ঘাড় এবং লেজের অঞ্চলে একাধিক ইন্ট্রাডাক্টাল ক্যালকুলির সাথে দীর্ঘস্থায়ী ক্যালসিফিক প্যানক্রিয়াটাইটিস রিপোর্ট করেছিল। এন্ডোস্কোপিক রেট্রোগ্রেড প্যানক্রিয়াটিকোগ্রাম একাধিক ফিলিং ত্রুটি সহ প্রসারিত অগ্ন্যাশয় নালী দেখিয়েছে।

চিকিৎসা:
অগ্ন্যাশয় নালীর পাথর দ্রবীভূত করার জন্য রোগীর ইআরসিপি এবং স্পাইগ্লাস লেজার লিথোট্রিপসি করা হয় এবং নালী সম্পূর্ণরূপে পরিষ্কার হয়ে যায়। নালী পরিষ্কার হওয়ার পর রোগীর প্যানক্রিয়াটিক ডাক্টাল স্টেন্টিং করা হয়। রোগী উপসর্গ মুক্ত, যেমন পেটে ব্যথা এবং স্থিতিশীল অবস্থায় ছাড়া হয়।

Chronic Calcific Pancreatitis Treated with ERCP and Spyglass Laser Lithotripsy

A 21-year-old young male patient from Bangladesh presented at PACE Hospitals with the complaint of upper abdominal pain radiating to the back, which he had been experiencing on and off since childhood. The patient was diagnosed with chronic calcific pancreatitis (3 years ago) and had a history of severe abdominal pain with hospitalizations every six months.

Diagnosis:

On admission in PACE Hospitals, the patient had undergone CT abdomen and reported with chronic calcific pancreatitis with multiple intraductal calculi in the neck and tail region with multiple parenchymal calcifications. Endoscopic Retrograde Pancreaticogram showed dilated pancreatic ducts with multiple filling defects.

Treatment:

The Patient Underwent ERCP and Spyglass laser lithotripsy for the dissolution of pancreatic ductal stones and the duct got cleared completely. After the duct is cleared, the patient underwent pancreatic ductal stenting. The patient is free of symptoms, like pain in the abdomen, and discharged in stable condition.
