As long as the key local manifestation of a myocardial infarction is understood as the coronary-occlusive death (necrosis) of a portion of the heart muscle (myocardium), then logically every patient should die. Necrosis of the myocardium means the irreversible loss of function, above all, the loss of contractile strength. Under the high pressure inside the ventricles, this should inevitably lead to rupture of the heart wall at the necrotic site.
And yet, in reality, only a small proportion of patients die. Today, the figure is much less than 5%. Why?
The answer is simple: a myocardial infarction is not merely the coronary-occlusive death of heart muscle. Rather, it is a special form of inflammation of the myocardium. Inflammation is the body’s defensive, compensatory response to injury. Its purpose is to replace necrotic tissue with connective tissue, which ultimately forms a stable post-infarction scar.
Thus, a myocardial infarction becomes a process of healing, unless the inflammatory response is disrupted. Only when this compensatory inflammation fails does necrosis progress unchecked and become fatal.
