Right coronary artery CTO with ambiguous proximal cap and diffusely diseased distal vessel. Retrograde crossing attempts failed, but antegrade crossing was successful true to true by using the lateral projection. The lesion could not be crossed by a balloon, but the 0.9 mm laser successfully crossed the lesion. Stents were optimized by using optical coherence tomography.
