The IDT has several topics that are address during the morning meeting. The PDPM requires the IDT to the patients who were newly admitted. The IDT review should include: Primary DX, skilled needs, medication review, care plan review, wound, infection control, pain assessment, and any items that place the patient at risk. The admission assessments should be reviewed and the care plan should be updated. If the discharge summary isn't present the team should request the record.
