Case no. 706:
a patient came with symptomatic apical periodontitis of previously initiated therapy on mandibular left 3rd molar. after taking a proper informed consent, i started the dental retreatment in a single visit as stated in the description below. But for sure, an accident happened, i broke the endodontic file at the apical third. So I did my best to perform maximum disinfection, and the result after 8 months follow up was awesome! the lesion had healed, and bone regeneration exist.
here are the procedure:
1. rubber dam: Sanctuary dental dam®
2. ultrasonic approach for file removal: gold tip from ultra-x, E3D tip, E6D tip, and E1 tip attached to ultramint pro device (eighteeth).
3. canal instrumentation using rotary file: Eflex blue UP TO 25/6% (eighteeth)
4. continuous ultrasonic activation: ultramint pro (eighteeth) with U-file (niti).
5. Diode laser: i laser 2 by Soga Dental
6. 3D obturation using fast fill and fast pack (eighteeth) and ceraseal bioceramic sealer (metabiomed)
7. coronal seal: low shrinkage stress flowable bukfill composite resin (palfique bulkflow)
8. core build up: high filler loading, medium viscosity flowable composite (palfique universal flow, medium flow)
9. Coronal layer: high filler loading, high viscosity flowable composite (palfique universal flow, super low)
10. gingivectomy: ACTTO 50A electrosurgery device
11. microscope: labomed prima
12. xray: nanopix
