The posterior drawer test is used to assess the integrity of the Posterior Cruciate Ligament(PCL). The PCL is responsible for resisting excessive posterior translation of the tibia (shin bone) on the femur (thigh bone).
The test is performed with the knee in 90 degrees of flexion and the foot stabilized distally. While palpating the joint line an anterior to posterior force is applied to the proximal tibia to assess displacement.
PCL and ACL injuries can be confused if the normal starting position of the knee is not identified. The anterior cortex of the tibia normally rest anterior to the end of the femur. In the video the tibia is then shifted posteriorly.
