Examples of items I want to get in this run: Arianna's Blood Vial or Monocular.
Examples of items I don't care about grabbing: Poison Knife or Pebble
I do all main game bosses, all DLC bosses, all trophy chalice bosses and additionally Hintertomb, Lower Hintertomb, Ailing Loran, Lower Loran and Isz Chalices/Bosses.
I get all normally attainable messenger items (there are some you can only get from pre-orders/special versions of the game).
As far as quests go, I do the ones that result in gestures, runes or special/unique items.
A special case is gascoigne's youngest daughter - one version of her quest yields white ribbon into red ribbon, but I prefer getting the rune, so I send her to the clinic and get the rune there.
