There is a powerpoint that discusses the aspects & steps of intubation and extubation. You will all see the checklist for the items necessary to intubate your patient. On that list is "3 endotracheal tubes- the 1 you think you need and a 1/2 size above & below. This 5 minute video will demonstrate how to select those tubes against a LIVE patient, how to make sure there is no leak in the tube and how to prepare a tie using an IV line cut to length and the back end of a 1 cc syringe. (There are several methods for securing an ET tube to a patient- my goal was to show you at least 1 reliable method here.)
FYI- remember.... there is no way to guarantee the exact tube diameter until you are in the mouth. If you decide you need a different size than the one you have in your hand... switch to the better sized tube! A poorly sized tube will hinder your ability to maintain your patient under anesthesia.
