How to Request Your Medical Records

Bernice Zander, Director of Health Information Management at Tahoe Forest Health System, explains the different ways patients can access their medical records. Records can be requested directly through the Tahoe Forest Health System website by completing the Authorization for Release of Information form, available in both English and Spanish. This form can be filled out electronically and returned with the contact details provided at the top. Patients may also call the Health Information Management office to request records, which can be released in several formats, including email, flash drive, fax, or by mail. In addition, outside providers such as physicians, insurance companies, or attorneys can submit requests on behalf of patients using their fax cover sheet.

Another convenient option is through the MyChart app. Patients can request records directly within MyChart, or use the “Share Everywhere” feature, which provides a secure website and access code for providers to view, save, or upload records into their own systems. This helps streamline communication and ensure continuity of care.
