NCFH’s webinar on community asset mapping will train health centers on how to successfully develop a community-asset map, the importance of partnerships, and how they can address social determinants of health (SDOH). NCFH’s community-asset map template will be provided to participants and can be used as a starting point to identify potential partnerships in their service area. Following the webinar, NCFH will provide office hours to assist in the implementation process, discuss lessons learned, successes, challenges, and strategies.
