2. Transhiatal (neck and abdomen incisions)
3. Ivor-Lewis (chest and abdomen incisions)
As well as some anatomic considerations for esophageal surgery (the left-right-left rule, how distal a tumor has to be to qualify for resection, and the favorite quiz question "what is the primary blood supply to the gastric conduit?").
