When LDL-C is extremely elevated, the next question is why?
Sitosterolemia, a genetic disorder that causes hyper-absorption of plant sterols, occurs relatively commonly in patients with low-density lipoprotein cholesterol ≥190 mg/dL.
In this video, we discuss the case of a 13 year old with sitosterolemia who presented with LDL-C of 679 mg/dL, and how he was treated to bring his LDL-C down more than 10 fold to 60 mg/dL.
The findings were published in the Journal of Clinical Lipidology appended to a study evaluating 200,000+ subjects. The authors found that 4% of subjects with LDL-C concentrations ≥190 mg/dL have plasma β-sitosterol (plant sterol) concentrations above the 99th percentile.
