Chief Complaint : Toothache
Secondary Complaints : Headaches, TMJ pain, clicking jaw, neck and shoulder pain, trismus, depression, pelvic pain and 33 other TMD symptoms
Treatment history : Severe toothache onset 6 months before first visit, toothache persisted after extraction. Botox treatment and splints did not help
Results : full recovery
