When you start a new save file, you spawn into a world with a random seed. I think there's 99,999 different seeds. All objects in the game (enemies, npcs, stars, coins etc) are in random locations in each stage. Each stage location is randomised, i.e. entering Whomps Fortress may take you to Dire Dire Docks. Which stage leads to what remains the same throughout the save.
You can exit a stage, but you lose a life if you do so, unless you exit to the castle lobby. This is why it's important to collect 1-ups as you run around.
As it's nonstop - you don't have to watch the star collection animation, and you continue running freely after collecting a star. The only way to leave a level is exiting stage, exiting to lobby, or dying.
The tracker you can see on the screen is for keeping an eye on what stage leads to where. It's also helpful for remembering if you've collected Mips or Toad stars.
Mips spawns at 15 and 50 stars, just like in vanilla sm64. The three Toad stars appear at.. I think... 12 stars for the basement Toad, then 35 stars for the two upstairs Toads. I'm not actually sure so I always get the two upstairs Toads after 50 stars lol.
The keys are not needed to go up/downstairs, so Bowser stages are useless. The doors to each stage however are protected under a random star count. This is why in the early game you definitely want to just follow the levels as you enter them, otherwise you're stuck testing random doors for a while.
