A 20 year old without a medical history suddenly collapses during a soccer match due to ventricular fibrillation. After successful reanimation, a coronary CT-scan is performed to evaluate for a cause. Indeed, the scan showed an explanation for the ventricular fibrillation, namely a malignant origin and course of a coronary artery. Specifically, the main stem has its origin in the right coronary cusp and then courses interarterially - between the pulmonary artery and aorta - towards the anterior interventricular groove. Because these arteries have high pressure, this abnormal course leads to compression of the main stem coronary artery, which in turn leads to myocardial hypoperfusion, ischemia and subsequent ventricular arrhythmias as in this case.
