2 of the most common reasons I hear for a failed cannula are

1) "I hit a valve"
2) "the vein spasmed"

I don't think either of these are good reasons to fail your cannula insertion.

I got through all the reasons in the video above, but also it is vital to check that you are actually in the vein.
Please use the tips in this video and consult your supervisor or a cannula expert to make sure you are actually in the vein.
