In this video, Dr. Corenman discusses pain regeneration in the spine. Pain receptors are dedicated nerve endings that permeate the body. Some are "raw" nerve endings with no attached receptors. Others have nocirecptors, , thermal receptors and chemical receptors. They are specifically designed to register tissue injury by chemical, pressure or temperature injuries. 

Why do we need pain? There are patients born with congenital insensitivity to pain. Patients born without pain feedback to their brain. Individuals commonly lose fingers and toes through burns and compression injuries due tot he lack of sensitivity to tissue injury. 

Dr. Corenman continues to speak about the following topics:loss of pain and proprioceptive receptors with paraplegics and quadriplegics, sclerotomal pain, Dwyer and Bogduk sclerotomal facet referral patterns, sclerotomal pain patterns, bone injury pain, and muscle generated overload pain. 

Historically, how did we figure out the sources of lower back and leg pain? Roentgen Rays discovered this in 1895. Finally, a window into the aging of the spine and living subjects that allowed knowledge of bony structures. Then, we had a cascade of advances in the diagnosis causing leg pain including Yeoman-Lancet who came up with the term “Piriformis Syndrome” in1928. Among many other important findings thereafter, this led to the connection between the lower back and causes of pain. Nylon sutures tied on various important structures during spine surgery: sutures were labeled, left long and protruded out of the incision.  

In 1991, Kuslich helped put together what causes pain and developed a pain connection. We see it with a lotof patients. Why do some people hurt more than others? Answer: facilitation, sensitization and psychological factors.
