5 Local Anaesthesia safe practice tips you MUST KNOW!

Local anaesthesia is an essential part of every doctors toolkit!

It is amazing how we can perform procedures painlessly and efficiently thanks to these amazing group of medications.

In this lecture I teach medical students some of the core principles that we learn during anaesthesia training... and some things that I think are advanced concepts but critical to safe practice and easy to learn!

Maximum volume formula

max dose/kg x patient weight = TOTAL DOSE.

TOTAL DOSE divided by concentration = MAX VOLUME

eg.
SCENARIO 1
70kg patient needs lignocaine for skin suture of skin laceration.
You have 1% lignocaine vials.

lignocaine max dose subcutaneously is 3mg/kg.
1% solution is 10mg/ml concentration.

TOTAL DOSE = 3mg/kg x 70kg = 210mg

MAX VOLUME = 210mg divided by 10mg/ml = 21ml



SCENARIO 2

100kg patient needs lignocaine for suture of multiple skin lesions.
You have 2% lignocaine with 1:200000 adrenaline.

lignocaine with adrenaline max dose subcutaneously is 7mg/kg.
2% solution is 20mg/ml concentration.

TOTAL DOSE = 7mg/kg x 100kg = 700mg

MAX VOLUME = 700mg divided by 20mg/ml = 35ml
