65 yr old smoker 65 yr old smoker, AWMI EF 35% , ongoing chest pain, Cag showed Calcific LAD, went radial route , proximal to mid 2 stents, distal stent not expanded well, 3 size nc balloon went till 22 atm, mid lad perforated, balloon inflated, Tamponade pericardiocentesis done, ping pong technique, femoral route 7Fr, bulky graftmaster 2.8 not able to pass through tortuous lad, 6Fr guidezilla, graftmaster deployed, sealed, no effusion, doing well now
