Circular Saw Jig Accuracy Issues

I made a jig for my circular saw to make straight cuts (I used an old video from Tom Silva of This Old House to make the jig). After making several cuts I realized that I wasn't getting the accuracy that I wanted so I looked at my jig a little closer and realized that there was a crook in my guide board. I thought it was because the plywood that I used was too thin (1/2 inch) so I tried using 3/4 inch and got the same result. Even though I sighted down the factory edge and it looked perfect, as soon as I cut a 4 inch strip from the 4 x 8 sheet and sighted down the same factory edge on the 4 inch strip it now has a crook in it. This video shows how I took the crook out of my guide board to make a more accurate jig.
