The darkness (Amara) thanks Dean by giving him what he needs the most.
Wandering through the woods, he meets an old face again, a dead face - Caroline.
Dean Winchester (Jensen Ackles) and Caroline Forbes (Candice Accola King) used to be together but when Dean found out that she was a vampire things started to fall apart. Dean tried to cure her but killed her instead.
Having her back means for Dean not only to face his guilt, but to face the woman who always understood, who always loved and forgave.
And it's not his guilt he has to overcome but the need to kill the vampire in her.
P.S. I hate that a lot of the clips I used aren't HD - I'm working on it.
