Evaluation and Management (E/M) documentation determines the medical necessity of the service. The documentation also determines the correct level of service. Practitioners can use either time or medical decision-making (MDM) to choose the level of service. This education will evaluate several examples of documentation. We will walk through the process of choosing the correct level of service based on the practitioner’s medical record.
