Taking a quick look at The Surgeon v1.5, a 1985 release for the Classic Mac.

I realized after making this that I actually made/used a 20KB CHD image instead of a 20MB one! But it was still enough to get the game to boot.

If you want a 20MB CHD emulating the Hard Disk 20. use:

chdman createhd -o macplus.chd -s 20769280

(You could probably get even closer and create a CHD that has the correct number of cylinders/heads/sectors, but it doesn't seem to be necessary)
