The "Eleven faced Avalokiteśvara" is a form of Avalokiteśvara bodhisattva with eleven heads.
The sutra contains the Heart-dhāraṇī of Avalokiteśvara-ekadaśamukha. In the sutra, the Buddha introduces and discussed how to practice the dhāraṇī and talks about the benefits and the incredible power of this dhāraṇī.
Ten main benefits:
One's body will be free from illness.
One will be constantly remembered by the Buddhas of the ten directions.
All wealth, clothing, and food will naturally be abundant without lack.
One will be able to defeat all enemies.
One will instill compassion in all sentient beings.
No poison or fever will be able to harm one.
No weapons will be able to injure one.
One will not be swept away by water-related disasters.
One will not be burned by fire-related disasters.
One will not experience untimely death.
Four additional results in this life:
At the time of death, one will see the Buddhas of the ten directions.
One will never fall into the hells.
One will not be harmed by any wild beasts.
After death, one will be reborn in the Land of Infinite Life (Amitābha's Pure Land).
Furthermore, the sutra also promises the state of Avinivartaniya (non-retrogression on the path of the bodhisattva, meaning one will not fall back to an earlier state of spiritual development).

Ili Milli Citi jvalam Apanāye. Svāhā. (Who bring the blazing understanding. Hail!).
Please note that there are several versions of the dhāraṇī and this rendition uses the most popular sung version popularized by Ani Choying Drolma (Namo Ratna Great Compassion Mantra 大悲咒梵音) and Imee Ooi (Arya Ekadasa-Mukha Dharani).

十一面观音(梵语:Ekādaśamukha Avalokiteśvara),又称十一面菩萨、大光普照观音, 是观世音菩萨的化身之一,是六观音之一。有十一个颜面,十一面之寓义,普遍说法表示“十地”,最顶一面,表示十地之上的阿耨多罗三藐三菩提。
此陀罗尼有多个版本,而我们采用的是由 Ani Choying Drolma (Namo Ratna Great Compassion Mantra 大悲咒梵音) 和Imee Ooi (Arya Ekadasa-Mukha Dharani 十一面观自在菩萨真言) 黄慧音 广为传唱、最为流行的唱诵版本。
