This is the fourth video in my series on patient care report documentation, this time diving into chest pain complaints. My agency uses the ESO platform for reporting, but the principles apply to all forms of recordkeeping. I emphasize using the FlowChart for treatment and following up with reassessment in the Vital Signs. At the end, a sample narrative illustrates how all the questions are woven into a DACHART narrative.
