*Medical Records the doctor can keep, we do NOT make copies.
*One Proof of Residency, you will need two if your address does not match your ID.
A form of payment for appointment as payment is due at time of service. We accept debit/credit cards, money orders, and checks made out to MCO.
If applicable, your Benefit Verification Letter for disability or DD214 if you are a veteran.
Follow up and recertification appointments: Please bring in your state issued ID or driver's license, any updated medical records, and your Illinois Medical Marijuana Card.
