Today, we have the 4K Ultra HD/Blu-Ray of The Matrix Reloaded (which I used to have on DVD before upgrading it) and the order for these formats are:

4K ULTRA HD:
1. Warner Bros. Home Entertainment logo
2. 4K Ultra HD menu
3. FBI Anti-piracy warning screen
4. "Piracy is not a victimless crime."
5. MPAA R rating
6. Warner Bros. Pictures logo (The Matrix variant)
7. Village Roadshow Pictures logo (The Matrix variant)
8. Title card

BLU-RAY:
Same as above.
