This time around we are going to discuss a case where we made a frontal tooth restoration. The restoration was made from zirconia. To do so, we needed to use a T base. We are going to discuss the different types of parts, and where to use each part.

In this case, a T-base with an angulated top was used. The reason for this is that the implant is buccally inclined, and unless we change the angulation, the screw channel will come right in the middle of the tooth.
00:00 Press on the gingiva on the implant.
00:33 Very old bridge that was made probably from zirconia with a gingiva foam composite material.
01:29 Those teeth had the root canal treatment a long time before the previous bridge was made.
02:49 These multi-units are screwed-retained.
03:56 The root canal was a long time ago.
04:57 It was made with CAD/CAM.
06:16 Here, we see a temporary bridge.
06:51 Would the old one get infected?
07:08 We made a temporary bridge, right on top of the implant that she had with the multi-unit that she had.
08:01 It looks much better now.
08:08 The multi-unit.
09:22 That's why we started producing the new color multi-units.
10:08 You can see these three are angulated multi-units. We took one, we colored it.
11:34 That's a cool idea. Making it the same color as the gingiva.
11:36 That's a great idea. This is in a rest position.
This case is a great example that you don’t always need to change the inclination of the screw channel to the palatal area. Sometimes (if the smile line is low), we can angulate the screw channel buccally, instead.
It’s important to use a T-base with a hex — an anti-rotational element between the implant and the T-base. This way, it won’t get rotated and unscrewed. Otherwise, if you use a round T-base for a single implant, it can be rotated by micro-movements unscrewing the crown leading to the risk of breaking it.
There are different parts for screw-retained restorations and other cases:
For cases with bridges, multi-units will be used.
For single crown cases, T-base will be used dues to their anti-rotational elements.
Additionally, there are two different types of T-bases:
With a hex (an anti-rotational element)
A round T-base, without a hex, which can be rotated according to the implant. A round T-base is used when there are parallel implants and you want to make a bridge on the upper part of the T-bases.
In short, V-type is good when there’s not enough space for metal structures, and D-type is good for any other case.
