What is your definition of a ‘design fail’?
IMO it is when the execution of the design falls short of the intended purpose, and as a result:
1. does not deliver as the application suggests it should, thus causing frustration and loss of efficiency,
2. or worse breaks under normal use applications, often requiring emergency repairs or parts replacement, OR, even worse
3. exposes someone to physical harm.
The design fail in this episode would fall into category Number 1 - - recurring frustration and loss of efficiency which ultimately requires that I modify the inadequate components with a superior design/fix that I will handcraft.
