Surgical Incision Healing: Rhomboid (Limberg) Flap Technique - Sometimes a longer incision and more stitches actually lead to better healing. The Limberg (or rhomboid flap) is a surgical technique first described by Russian surgeon Alexander Limberg (1946) that continues to influence how surgeons close certain wounds. The procedure works on a simple principle: incisions heal side-to-side, not end-to-end, and tension matters tremendously in wound healing. When standard suturing would create too much tension, the Limberg flap offers a geometric solution #WoundClosure
The #suturing technique involves creating a rhomboid-shaped incision around the defect, then extending a cut from one corner to create a matching flap. This flap is rotated into position like a puzzle piece, transforming what would have been a straight line under strain into a relaxed zigzag pattern for more natural healing - each angle is precisely calculated to ensure equal sides and perfect tension distribution. This redistribution prevents pulling on important structures and creates a Z-shaped scar that heals better #PlasticSurgery
Limberg flaps are particularly valuable in areas where skin is less mobile or where preserving function is critical, commonly used in reconstructive surgery after tumor removal, pilonidal sinus treatment, and for some facial reconstruction where cosmetic outcomes are paramount. The technique was later refined by French surgeon Claude Dufourmentel and American surgeon Jerome Webster, each adding modifications to improve versatility/outcomes. Beyond improved healing, the flap maintains excellent blood supply through preservation of the dermal-subdermal plexus, reducing the risk of tissue necrosis.
