Highly effective modulator therapy (HEMT) has been available for eligible people with CF for more than three years. We discuss how airway clearance techniques and patient engagement have changed during this period across pediatric and adult care and where we are going next. We describe how centers are trailing new airway clearance techniques by comparing passive and active treatments among pediatric and adult patients. With an overall reduction in engagement in airway clearance therapy (ACT) amongst adults, we hear from a large adult center on their experience re-engaging patients in treatments especially those who report being non-productive. Here we examine how new education and assessment methods are being used, alongside some case study examples and the potential difficulties we are still facing.
