In regulated industries, CAPAs (Corrective Action and Preventive Actions) are essential elements to investigate and fix issues.
But far too often, the investigation ends at the finding of cause as "Human Error" or similar terminologies.
"Human Errors" can be a symptom of an issue, but can never be the root-cause themselves. Hence, they have no place in CAPA reports.
So, how can we handle such issues featuring "Human Error" as an element?
