Retrieving an impacted 2nd bicuspid.
Is it possible?
This patient came to us with a difficult goal. Usually retrieving an impacted tooth is not a big problem. You open a gap for it to be able to either wait for it to extrude on its own, or you pull on it with a chain. That depends on the time and urgency of the case and your patients co-operation.
After looking at the Panorex we saw that the tooth was heavily mesial rotated to 90°. Just opening a gap wouldn't change its position. So we should extract it right?
No! After opening the space and making sure the molars roots do not touch the impacted 2nd bicuspids crown, we chose to pull on it with a chain that was placed by a colleague of ours. With that chain we were able to actively mount it to our orthodontic brace and put it in its right position to then finish the case with opened gaps for planned implants.
The patient is happy, and we are too!
