Sharing ("altruism") is the natural state, selfishness is abnormal.
Selfishness involves the inability to perceive the externality and separateness of object (the absence of representation of sharing partners, internal objects, or selfobjects or when the selfobject is the alienated, depersonalized immature or unintegrated self).
Healthy, normal people have moments of solipsism but it is the permanent state of the narcissist and the psychopath.
I suggested that the only way to conclude that pathological narcissism has been mitigated or ameliorated is by observing the narcissist in social settings. If the individual no longer meets the criteria for a diagnosis of NPD (which is diagnostically defined in relation to other people), it is safe to assume that the underlying pathology is gone.
BUT
ONLY if we observe the narcissist over an extended period of time (many years). It has to be longitudinal. Otherwise, such a conclusion would be speculative at best and wishful thinking or self-serving at worst.
We also have to always take into account lifespan impacts: certain antisocial/dissocial and abrasive behaviors naturally abate and remit with age.
