Welcome to a new series of Plague Inc: Evolved mega-brutal guides, remastered for the 2025 World Update!

The bacteria is the most "vanilla" of all the disease types and serves as the perfect introduction to the mega-brutal difficulty. This strategy will teach you how to balance infectivity and severity, read the map, and time your lethality ramp-up. You can beat this scenario with no genes, but your score will obviously be lower. NOTE: Even if you follow this guide perfectly, there will always be some randomness and you may have to restart if you get unlucky. It's all part of the game!

Paranoia
Once spread to Russia, get:
Pneumonia
Cold Resistance 1
Cold Resistance 2
Otherwise, continue with:
Air 1
Water 1
Air 2
Water 2
Extreme Bio-Aerosol
Drug Resistance 1
Livestock 1 (optional)
Genetic Hardening 1
Genetic Hardening 2 (unless all islands infected)
Hold until all islands are infected or cure has reached 40%
Necrosis
Pulmonary Fibrosis
Total Organ Failure
Coma
Seizures
Insanity
Paralysis
Genetic Reshuffle 1
Genetic Reshuffle 2
Genetic Reshuffle 3
If struggling with islands like the Caribbean, consider picking up a level of heat resistance instead of livestock transmission.
