이명은 크게 타각적 이명과 자각적 이명으로 나누어 볼 수 있습니다.
타각적 이명은 다른 사람이 들을 수도 있는 이명으로, 귀주변의 혈관이나 근육에서 발생하는 내 몸에서 나는 소리가 귀로 전달되어 소리가 들리는 것으로, 일반적으로 일정하고 규칙적인 소리로 느끼게 됩니다.
이명의 대부분은 자각적 이명인데, 자각적 이명은 다른 사람들은 듣지 못하고 본인만이 소리가 느끼는 경우로 환자들은 귀뚜라미나 매미가 우는 소리, 바람소리, 삐, 윙, 쏴~하는 소리 등으로 다양하게 표현하곤 합니다.
이러한 자각적 이명의 가장 흔한 원인은 난청에 의한 것으로, 중년 이후 청각기관 퇴행성 변화나 손상을 먼저 생각해볼 수 있고, 근래에는 큰 소음에의 노출에 의한 이명도 증가하는 추세에 있습니다. 그 이외에 청력을 떨어뜨릴 수 있는 중이염, 메니에르병, 외이도염, 귀지 등도 원인이 될 수 있습니다.
또한 이명은 고혈압, 당뇨병, 갑상선 질환, 약물, 두경부 손상 등과 연관되어 나타날 수 있습니다.
이렇듯 자각적 이명의 원인은 매우 다양해서, 원인을 뚜렷하게 밝혀내기 어려운 경우가 많습니다.
