In this informative video, Dr. Mukesh Aggarwal, M.D., renowned Trichologist, Author, and Director of the World’s 1st Ayurveda Hair & Skin Clinic – VHCA Ayurveda, discusses one of the most commonly asked questions:
“Can Alopecia Areata lead to Scarring Alopecia or cause permanent hair loss?”
Hair loss, especially patchy hair loss, is a concern for millions around the world. While it can be alarming to see bald patches appear suddenly, it’s important to understand that Alopecia Areata and Scarring Alopecia are two different conditions with distinct causes and outcomes.
🔹 What is Alopecia Areata?
Alopecia Areata is an autoimmune condition where the body’s immune system mistakenly attacks its own hair follicles, leading to patchy hair loss.
It is a non-scarring type of alopecia, which means that the hair follicles remain intact, allowing hair to regrow naturally once the immune imbalance is corrected.
🔹 Alopecia Areata is:
⦿ Non-contagious (it doesn’t spread from person to person)
⦿ Often triggered by genetics, stress, hormonal imbalance, or poor lifestyle
⦿ Reversible in most cases with proper Ayurvedic and medical treatment
🔹 Difference Between Scarring and Non-Scarring Alopecia
Non-Scarring Alopecia (like Alopecia Areata):
⦿ Hair follicles remain healthy and intact
⦿ Hair regrowth is possible
⦿ Caused by immune or hormonal imbalance
Scarring Alopecia:
⦿ Hair follicles are permanently destroyed and replaced with scar tissue
⦿ Hair regrowth is not possible
⦿ Usually caused by severe inflammation, infections, or trauma to the scalp
🔹 Grades of Alopecia Areata
1️⃣ Grade 1 – Mild: Small, single patch of hair loss
2️⃣ Grade 2 – Moderate: Multiple patches appearing on the scalp or beard area
3️⃣ Grade 3 – Severe: Extensive hair loss involving large scalp areas or entire scalp (Alopecia Totalis)
Even in severe grades, the condition remains non-scarring, meaning that with the right treatment, hair regrowth is still possible.
🔹 What is Scarring Alopecia?
Scarring Alopecia (also called Cicatricial Alopecia) occurs when inflammation damages the hair follicles beyond repair, replacing them with fibrous tissue. This leads to irreversible hair loss.
It can be caused by:
⦿ Chronic scalp infections
⦿ Autoimmune disorders (like lupus erythematosus)
⦿ Prolonged untreated inflammation
🔹 So, Can Alopecia Areata Become Scarring Alopecia?
The answer is — generally, NO.
⦿ Alopecia Areata does not turn into Scarring Alopecia in most cases.
⦿ However, in some rare and chronic cases, if inflammation persists for a long time without proper treatment, it can cause partial follicle damage, leading to limited permanent hair loss.
This is why early diagnosis and proper treatment are essential.
