In this penultimate episode, I concentrate on periorbital treatments, restoring the transition from the eyelid to the cheek, making the tear trough appear higher up, and reducing the deep shadows above Em’s eyes.

Watch as I administer tear trough filler, moving through the subcutaneous depth, treating deep behind the orbicularis oculi muscle with a micro droplet technique, being careful to avoid treating the nasojugal groove.

Next I use a supraorbital treatment to reduce the height of the shadowing above the eye. The tail of Em’s brow already has beautiful support, so I avoided touching this, instead focusing on creating volume and reducing hollowness underneath the orbit.

I pair this treatment with some A-frame filler to further address the hollowing here, and restore some youthful volume. A cannula is always best here, due to the vessels in this area.

Next, I treat Em’s pyriform fossa to improve the volume of her medial cheek and help with the transition across her nasolabial fold, as well as to slightly elevate her upper lip.

Watch as I locate and inject the deep pyriform space with a 22G cannula, using a deep projecting filler. You’ll see how I move through the different layers of the face, from the skin to the subcutaneous fat, through the SMAS with a 60º angle. You’ll see the twisting motion I use when encountering resistance in the SMAS, before moving behind the LLS muscle.

Once we’re past this LLS, I progress through the deep medial cheek fat into the target of the deep pyriform space.

👄 Next, we want to balance Em’s aesthetic by increasing the height of her lip using a vertical tenting lip filler technique. In doing this, we rejuvenate her perioral region more generally, and also address her ‘smoker’s lines’.

💎 For this lip filler treatment I aim to create as much natural-looking volume at the Cupid’s bow as possible, without looking over-projected. We stay within the lip compartments, respecting these anatomical boundaries, guided by the latest evidence found in the Global Evidence Matrix (GEM© by Harley Academy) app.

We break for a split-face assessment of Em’s treated versus untreated sides, and an ultrasound review conducted by Dr Joanna Hackney, Head of Medical Services at Harley Academy.

Lastly, I round off this episode by disconnecting Em’s dermis from the orbicularis oris muscle, adding a sheet of hyaluronic acid, to further improve her perioral lines. Then finish by treating the mandibular angle, sculpting her jawline with a highly projecting filler, to enhance the results at the end of her treatment sequence.
