If the donor does not initial the seal or refuses to initial, the collector must indicate it on the CCF in the “Remarks” section. This is not a refusal to test.

If the donor refuses to sign Step 5 on Copy 2 the donor certification section, this is not a refusal to test. Print the donor name where indicated in Step 5 on Copy 2. Make a note in the “Remarks” section on Copy 1 that the donor refused to sign. Complete the collection, again this is NOT a refusal to test.

If a collection problem resulted from the omission of required information, the collector must, as the person responsible for providing that information, supply in writing the missing information and a statement that it is true and accurate. Always add in the remarks section anything out of the ordinary, including when donor refuses to initial the form or sign Copy 2 the MRO copy of the CCF form, the donor certification section.
Always write the donor name on Copy 2 of the form in any collection event where you are completing a CCF form.

Once the urine vials are filled with urine, the employee is then requested to initial the specimen seals. The employee must be present to observe the sealing of the specimen bottles. If the employee fails or refuses to initial the seals, the collector must note this in the “Remarks” line of the CCF and complete the collection process; this is not considered a refusal to test.

If the employee refuses to sign the form or provide date of birth, printed name, or telephone numbers, the collector must make a notation on the "Remarks" line to that effect and complete the collection. If the employee refuses to fill out any information, the collector must, as a minimum, print the employee’s name in the appropriate place. This does not constitute a refusal to test.

Remember a refusal to initial the specimen labels or refusal to sign the CCF form copy 2, MRO copy is not a Refusal to Test.
