2024 Chronic Care Management (CCM) CPT Codes, Billing, and Reimbursements

Learn about the 2024 CMS Final Rule updates for CCM. Chronic Care Management is a Medicare program for patients with two or more chronic conditions. We'll examine the CPT codes for Non-Complex CCM, Complex CCM, and Physician-Driven CCM, including codes 99490, 99439, 99491, 99437, 99487, 99489, and code G0511 for Rural Health Clinics (RHCs) and Federally Qualified Health Centers (FQHCs).
