Living with vision loss presents unique challenges, particularly when it comes to remaining independent at home. Care Managers and Occupational Therapists play distinct yet complementary roles in helping individuals manage these challenges. From navigating healthcare systems and securing resources to adapting the home environment for safety and accessibility, their combined efforts are essential in assisting clients to stay safe, empowered, and engaged in daily life.

Ellen Platt brings over 35 years of experience as a Certified Rehabilitation Counselor and Certified Professional Aging LifeCare Manager. Her practice centers on comprehensive care management for individuals facing catastrophic injuries, chronic illnesses, and disabilities. She founded The Option Group to better serve the needs of vulnerable adults and children, while easing the burdens that caregivers and families face. Ellen and her team develop individualized care plans, offer ongoing case coordination, and advocate for quality of life, safety, and dignity. Their services cover Maryland, Delaware, and Pennsylvania.

Home Safe for Seniors

Amy Young is a licensed occupational therapist with over 25 years of experience promoting safety and independence for individuals with disabilities. She founded HomeSafe for Seniors to provide home safety and accessibility consulting, initially for older adults but now serving people of all ages with physical challenges. Amy specializes in identifying home modifications and adaptive equipment to help clients stay safely in their homes. She holds certifications as a Certified Aging-in-Place Specialist and has advanced training in home modifications and geriatrics.

Home Safe for Seniors is an initiative that supports older adults by improving the safety and accessibility of their living environments. The organization helps individuals remain in their homes longer and more confidently through assessments, recommendations, and home modifications. They are passionate about creating practical solutions that reduce fall risks and empower seniors to age in place with dignity.
