Goal
The patient will report reduced discomfort . The patient will maintain regular bowel movements and life style habits
PLANNING
Do history collection and physical examination
Encourage a high- fiber diet
Recommend an increase in fluid intake
Regularly assess bowel movement patterns, stool consistency, and patient comfort
Monitor the patient for signs of dehydration
Teach the side effects of certain medications and their impact on bowel function
Assess the stress level and mood changes in patient .
Collaborate with a dietician for dietary plans
Refer the patient to physical therapy if mobility is an issue
EVALUATION
The patient reported reduced discomfort . Also the patient regained regular bowel movements and regular life style habits
Administer medication as per doctors order
