GOUT/ GOUT ARTHRITIS - NOTES

Gout is metabolic disease, caused by increased level of uric acid (Hyper-urecemia) that results in acute arthritis.
Uric acid crystals (monosodium urate crystals) deposits in joints, tendons and other joint structures.
Uric acid is metabolic waste that forms after digestion of PURINS
Normal serum uric acid level 3.5 to 7 mg/dl (MALE) or 200 to 450 micro-mol/lit.
Normal serum uric acid level 3.0 to 6 mg/dl (FEMALE)
 
Risk factor/causes
·         Increased serum urate/ uric acid, Old age, genetic cause.
·         Uric acid level increase 2 times more can cause renal stone. ( more than 9 mg/dl)
·         Hyperlipidemia, obesity, excessive alcohol.
·         After chemotherapy and radiation therapy :- due to cellular breakdown, DNA breaks in purine & purine breaks in uric acid.
·         Purin rich foods - Meat, fish, liver, seafood, oat meat, spinach, cauliflower, mushroom, peas, beans, lentils, sweetbreads, high fat milk, high sugar  etc. Purin rich foods have high risk to develop gout.
 
Symptoms
·         Pain, swelling ,redness
·         Most commonly affected – big tone (first meta-tarso-phlangeal joint)
·         Tophus of tophi formation
Topus/ Tophi
·         It is deposition of uric acid crystal in people with hyper-urecemia. It is characteristic of gout – arthritis.
·         Urine acid deposition occurs under the skin that causes bone and cartilage destruction. Tophi can be found around the joints or in the bursa such as big toe, elbow. Tophi caused by crystals of ‘monosodium urate’
Diagnosis – Synovial fluid analysis (Golden standard) & uric acid test
 
Treatment
·         Avoid alcohol and other Purine diets, Take plenty of fluid.
·         Avoid meat, fish, and chicken.
·         Include Purin lowering diet such as citrus fruits, lemon, pineapple, water, apple, omega 3 fatty acid foods such as walnut etc.
·         NSAIDs – Ibuprofen, Celecoxib for pain
·         Colchicines (for gout pain)
·         Methyl prednisone.
·         Allopurinol/Zyloprim (uric acid lowering drugs)-D.O.C
 
Note -  Gout also called as “Disease of king”, because rich foods have high concentration of protein and fat. (Benjamin Franklin, tomas Jefferson, Sir Isaac neuton, Charls Darwin, king hennery all suffer from gout.
