Impact of Renal Function on Iodine-131 Biological Half-Life in Thyroid Cancer Patients

ABSTRACT
🧠 About this Study:
This video summarizes a systematic review and meta-analysis exploring how kidney function, measured by estimated glomerular filtration rate or eGFR, influences the biological half-life of Iodine-131, or I-131, in patients with differentiated thyroid carcinoma, DTC.
⚗️ What We Did:
We analyzed data from 13 studies involving 1,811 patients to understand how changes in kidney function impact I-131 clearance. Using meta-regression and pharmacokinetic modeling, we developed an evidence-based dose-adjustment model for radioactive iodine therapy, RAIT.
📊 Key Findings:
There is a strong inverse relationship between eGFR and I-131 half-life, with r = -0.678 and p less than 0.001.
Patients with lower kidney function retain I-131 much longer.
The model suggests reduced I-131 doses of 59–74 percent for patients with moderate kidney impairment, with eGFR between 45 and 59 milliliters per minute per 1.73 square meters.

Kidney function plays a major role in how quickly the body clears I-131. Adjusting the dose based on eGFR could make radioactive iodine therapy safer and more personalized, helping prevent radiation overdose and reducing risks such as blood toxicity and secondary cancers.
