Co-hosted by the Massachusetts Executive Office of Aging & Independence (AGE) and the MassTech Collaborative’s Mass Caregiving Initiative, this webinar introduced the Dementia Care Planning Toolkit, a new resource designed to support person-centered care planning for individuals living with dementia.
Developed by the Massachusetts Advisory Council on Alzheimer’s Disease and All Other Dementias, the toolkit is intended for people with dementia, their care partners and families, community providers, and healthcare professionals.
